Central Clinic according to the month roster
Reception of Patients: Enquire the patients and clients of their need and requirement, direct them to the registration office for registering them if on the 1st visit. Categorize according to the illness, Syphilis /Gonorrhea/HIV. Refer the regular visitors to particular appointment room.
HIV room
My task here involve register the new patients and open history file for each of them.
Refer the (clients) patients to the particular Doctor in the HIV clinic.
I assist the doctor and the consultant veneriologist at internal and speculum examination of the patients and obtain High l swabs for microbiological assay (Chlamydia, Tricomonas lis, HPV)
I Keep records in the personal file of HIV+ patients securely.
According to the clinic visits, I maintain the records of clinical examination and various other reports according the chronological order for further reference, when necessary.
At this station I withdraw blood for CD4-counts and other necessary investigations done in the Laboratory.
When I lookafter the HIV room, I personally present and attend to them.
I, maintain drug charts / and issue orders, and keep records of them according to the prescriptions made by doctors and consultant veneriologists and Microbiologists..
As the HIV-clinic nurse, my prime responsibility is to take charge of each, Patient’s records confidencially and assure of them getting correct drug dosages without interruption.
I achieve this task with the continuous communication with the indoor dispensary (Pharmacy) and the Chief Pharmacist.
I Keep continuous co-ordination between the clinical staff of the infection diseases hospital (IDH) about admitted cases (AIDS)
I ensure delivery of essential drugs to such patients who are warded in other hospitals according to the requests made by the hospital from the central clinic.
I keep an eye on the HIV status of such patients in medical /obstetrics /Paediatrics specialized hospitals with infection control units of hospitals and assist follow up of the HIV+ mothers and neonates later in the central clinic. I maintain records, with responsibility.
I perform Coordination of HIV+ mothers and babies who visit the central clinic from remote areas.
I have been in contact with those units where the HIV+, ANC mothers being looked after with regard to the prevention of transmission of infection to the baby and safe delivery of drug (Nevirapine) to the needy.
As my special interest rests in prevention of mother to child transmission, (PMPCT) I am in touch with the safe delivery and post partum follow up of this babies even when I am not on the HIV Room duty roster.
IV-room (bleeding/Injection) Nurse
On the 1st visits, I attend to the patients for withdrawal of blood for various investigations like,
TPHA/VDRL, HIV-ab and Hep-Bs-Ag:
For the HIV+patients I attend to withdraw blood for CD4 counts, Viral Load FBC Toxoplasma, CMV, HepC antibodies and FBS, PPBS, serum electrolytes.
I label and dispatch the specimens for the clinic Laboratory and side (MRI/RIA) Labs.
During treatment of the patients I have to do sensitive tests under doctors’ instructions and give injections of IM, IV-drugs.
For syphilis patients, I am supposed to inject Benzathene Penicillin deep-IM after-ST and for Gonorrhoea Cefuroxime-IM and so on.
I have to attend to patients who succumb to drug reactions during treatment and initiate resuscitation of them, I have to look after and monitor vital functions and give emergency care to them (RR/BP/Pulserate) or admit them to the ETU in A&E department of the hospital.
Medical Examination Room Nurse.
I work as a member of the Nursing team in the medical examination (check up) room meant for government recruitment of all the new recruits, in the Western Province.
All the new recruiters of Armed forces and police personnel undergo their fitness eligibility, blood pressure measurements (Physical) examination at this station.
I measure height, weight (BMI) as well as do withdraw blood for HIV, VDRL, WBC, /DC, and refer them for further tests done in the chest, eye clinic in other institutions, in fitness assessment.

With all the above function performed by me at the central clinic in Colombo, I am responsible to the Nursing in charge, MO-IC and the director of the STD/AIDS control programme of the Ministry of Health.
I am fully satisfied with the work entrusted with me and I work to the satisfaction of my superiors and I assist to control the transmission of HIV as well as other STDs.
Past work experience.
Antenatal clinic nurse - From the 1st week of July 2003 to 1st July 2004
De Soysa Maternity Hospital, Colombo-08.
My 1ry duty as a ANC clinic nurse, falls under the in charge nurse of the clinic include,
- attend to ANC mothers who get registered in the clinic
- Categorization of ANC mothers according to the parity and other associated factors, such as primi / primi breech, previous uterine scar etc and divide the patients in to broad groups as High risk/ low risk, and soon.
- Prioritize of the transferred mothers as referred patients from other institutions, and take over them to the clinic or admit to respective units.
- Assist the medical staff at clerkship, and history taking from the mothers and examination as well.
- Record keeping of the patients for further follow up
- Supervision of the junior nursing/midwifery staff doing height / weight and BMI measurements.
- Teaching the nursing students and Lecturing to them.
- Supervision and carrying out blood withdrawal for VDRL/Hb%, Blood grouping and Rh and other test which had been ordered
- Labeling and dispatching the specimens to the respective Laboratories
- Receiving and distribution of the reports according to the reference numbers of the patients.

Gynae clinic nurse.
My Functions include.
Reception and registration of the patients on arrival. Assist with obtaining histories of the illness from the patients.
- Assist speculum examination and obtaining high l samples and swabs for further investigations.eg – microbiological cultures / ABST.
- Maintenance of records of patients with regard to progress of the symptoms , who are Climecteric on drug treatment, for e.g.-

- who are waiting for various Gynecological surgeries due to prolonged waiting lists
- Obese patients on Nutritional therapy and weight reduction mannoures before treatment.

I work on above patients and maintain records for evaluation of the drug treatment and counsell following patients who live with problems.

Reference: of patients to other clinics in various IIIry care hospitals, when they need integrated care for eg. Certain patients underwent difficult labour / Hysterectomy following labour / (caesarean Hysterectomy) and who have had ICU care following delivery.
Post natal / Neonatal clinic nurse.
My duties include,
- Maintain records of such mothers, who are being followed up at postnatal clinic,
- Assist at neonatal examinations and maintenance of records in the postnatal clinic.
- Assist and supervision of blood withdrawal from mothers for further investigations who are diabetic and hypertensive
- Provision of immunization facilities and maintenance of cold chain according to the criteria laid down.
- Counseling and reference of identified patients for health education unit for postnatal exercises, perineal care advices
- Advice on/ implement family planning with OCP, IUCD, Norplant, implanon and DMPA inj who request FP.
- Withdrawal of blood from neonates for various investigations and dispatch them to the Laboratories.
I have performed covering up duties of the Antenatal, Labour ward/ postnatal and Gyane ward nurse on temporary basis during my tenure.
I have assisted l deliveries, vacuum extractions and forceps deliveries, suturing episiotomy and l tears.
I have done Labour ward preparation of patients who were planned for the pre labour insertion of suppositories, insertion of l supportness giving perineal care etc.
I have performed pre Labour/Pre LSCS preparation of antenatal mothers, insertion of bladder catheters, giving premeditation.
Operating Theatre/ Scrub Nurse/ Recovery room Nurse
From 1st week January 1996 to May 2003- De Soysa Maternity Hospital.
I perform registering of the patients who attend the surgeries
I have supervised the theatre preparation as the in - charge of the shift.
I have been working as the team leader of nurses of about 10-12 nurses on a shift. I have arranged duties of the nurses who were on the duty roster.
I have supervised theatre attendant staff and supervised the work of about 15 of them onshift.
- I have attended to patients who needed premeditation prior to the general/spinal anesthesia, and given oral Na citrate /IV-atropine treatment
- I have assisted at epidural analgesia
- To my account I had been assistant nurse at various elective and emergency LSCSs done in various maternal and fetal indications.
More over I have assisted for hours who underwent TAH and other surgical procedures just after -emergency LSCS and the patients who collapsed post partum.
I have attended to and assisted neonatal resuscitation which is part and parcel of theatre assistant nurse’s duty. During these procedures I have performed cardiac massage as well as injection of live saving drugs like adrenalin.
I have assisted at failed l delivery who were terminated by LSCS
I have performed my duties assisting consultant Obstetricians and Gynecologists attending major surgeries like,
-TAH +BSO done for various uterine pathologies
VH and R, done in middle aged as well as in old who have had descent of uterus

To my account I have assisted many a thousand minor gynaecological procedures like EUA+D&C, Cervical biosy, cervical cerclarge, D and E, ERPC and B/L-LRT-under GA and epidural anesthesia.
Imanaged Collection of specimens and dispatched them to the Path Labs and record keeping of the surgical operations too.
Sterilization / Infection Control/ Recovery Room - Nurse
As an in charge of a theatre shift, I have been responsible for overall sterility of the theatre suite.
I had been successful in dispatch of used instruments and linnen to the CSSD and reception of
- sterilized material for smooth functioning of the theatre.
I looked after proper functioning of the Anaesthetic machines /laparoscopy/uteroscopy pulse oxymeter, ventilators and continuous supply of N2o/O2/N2.
I have been responsible of proper storage and supply of dangerous drugs like pathidine/ morphine
- under strictvigillance and supply of enaesthetic drugs suxamethonium, Tubocurarine, Thiopenitonium Na.
I have worked to the TQM of the theatre function.
Recovery Nurse. ; During my shift as a theater nurse I have worked in the recovery room. I have lookafter Patients who underwent LSCS and other Gynecological operations, Under GA, during recovery; I have mono record level conciseness, SBP, Pulse rate, pulse pressure as well as watching for bleeding. I have to suck out secretions vomits and monitor and record keeping – delayed recovery and inform the anaethetist urgently.
